USER AGREEMENT

1. Any reporting of the results of the BEAP will give appropriate credit to the authors of the survey/instruments.

2. Raw data from the completed surveys will be provided to the authors to allow for development of the database and to insure further refinement of the BEAP.

3. Each BEAP survey must be used in its entirety. Users may administer additional questions or forms to accompany the BEAP questionnaires as they deem appropriate, as long as they are clearly labeled as separate and the BEAP instruments remain intact. The questionnaire is not to be edited in any way. The additional or supplemental questions must be tabulated locally by your program.

4. If hard copies of the instruments are used (as opposed to online versions), the program agrees to use only those scannable answer sheets provided by the research team.

5. The program agrees to remit the charges for the requested number of answer sheets. No materials or data analysis will be provided until the charges are paid in full.

6. If hard copies of instruments are used, the program agrees to photocopy only enough Exit Questionnaire sets, and Graduate Questionnaire sets to accompany the related scannable sheets indicated on this Use Agreement.
